DECLARATION AND CONSENT
I, the undersigned, hereby declare that the information given by me in the Application For Employment is correct and true. I fully understand that any false information given may render my application ineligible for consideration and that if I am offered employment, I may be subject to disciplinary action and liable for dismissal by ORIX Leasing Malaysia Berhad [Business Registration No.: 197301002250 (15741-D)] and its related companies as defined under Section 7 of the Companies Act 2016 (collectively referred to as “OLM Group”).

Additionally, for purpose of considering my application for employment, I hereby voluntarily and unconditionally consent to / authorise OLM Group and its officers to process my personal data (including my sensitive personal data) as defined under the Personal Data Protection Act 2010 and its amendments, that are provided by me and / or that which OLM Group may obtain from or through public or other sources, in the following manner:-
- to verify and process my personal data as OLM Group deems fit and necessary;
- where appropriate, to disclose any or all my personal data to the companies within OLM Group for considering my application for employment and to any party with a need to know as OLM Group may at its sole discretion decide;
- for OLM Group to disclose any or all my personal data to any credit reporting agencies which services OLM Group may subscribe to and for the same to further process my personal data as required under the Credit Reporting Agencies Act 2010;
- to make personal reference check with my referees as well as my previous employer(s); and
- for OLM Group to keep any or all my personal data for future references.
In relation to (e) above and during the period when my personal data are kept by OLM Group, I am aware of my rights as a data subject to access, correct, and update my personal data by request in writing to the Head of Group Human Resource Department of OLM Group at Level 18, Vertical Corporate Tower B, Avenue 10, The Vertical, Bangsar South City, No. 8, Jalan Kerinchi, 59200 Kuala Lumpur.
